Four support worker posts. 2 @ 15hours and 2 @ 14 hours

Can you support the team manager to development, deliver and evaluate day opportunities life skills & social programmes supporting people who have a learning disability, long term condition, mental health challenges or Autism. Supporting their journey towards independence, choice rights and inclusion through identified personal outcomes.
